A man was using a machine that folds metal sheets. The safety guards had been took off and put behind the machine for several years. When he was using the machine his finger came in contact with a moving part severing his finger in half. As a result of the injury at work he had part of his finger amputated, the surgery caused him to be off work for two months. An investigation was carried out which stated that the floor manager was responsible for Heath and Safety though he was not given time to carry out any risk assessments or change the state of the machine. The company received a fine £4,000.
