Subject: Man left seriously injured at work after safety failures lead to fuel tank explosion
A man in his 20′s has suffered burns to his face and wrist and was in intensive care for two days when sparks from a disc cutter he was using ignited fuel vapor in the tank.
As well as the worker being injured, a number of nearby properties and vehicles were damaged by flying debris. It was discovered that the tank had not been properly cleaned and checked as flammable residues remained, causing the explosion.
A number of safety failings led to the accident costing the firm £5,000.
