A 49 year old woman was crushed to death by a forklift truck.
The accident happened when she became trapped under a forklift truck that was unloading palettes of snacks at a warehouse. An inquest heard that the woman had had several previous near-misses.
Paramedics had found that her heart had stopped when they arrived and she was not breathing. She was immediately rushed to hospital but later died from multiple fractures and internal injuries.
