A three year old girl has been sadly involved in a fatal accident whilst playing at nursery.
The girl died after becoming tangled up in a 16-metre rope on a slide she was playing on that was out of sight of nursery workers. However, the manager of the nursery claimed she never saw anything that concerned her during her safety inspections of the slide.
