An engineering firm have been sentenced following a fatal accident at work where a 46 year old man was killed.
The accident happened when the 46 year old was carrying out maintenance in a vacuum when the main isolation valve closed suddenly trapping his head. He was pronounced dead at the scene.
After investigations were carried out it was reported that the company had failed to act for several years on repeated recommendations from their own Corporate Health and Safety Manager; and had failed to adequately train and supervise maintenance staff. As a result of the fatality, the company was fined £300,000 and ordered to pay £77,500 in costs.
